In Indiana, abortion doctors must complete a Terminated Pregnancy Report (TPR) for each individual abortion performed. TPR’s are public record and copies may be requested via the IN Department of Health. See the photos below showing a real TPR from 2022.
Each TPR contains the following information:
- Facility name, address, city, and county,
- Patient’s age, marital status, education level, race and ethnicity,
- Previous pregnancies, births, and terminations,
- Sex of the fetus,
- If the fetus was viable, delivered alive, and if a pathological exam was performed,
- Preexisting medical condition’s of the patient,
- If the termination resulted in maternal death,
- Procedure/s that we used and if the required educational information was given,
- Date of woman’s last menses, physician estimated gestation, post fertilization age of the fetus, and method of determining fetus age,
- If there is a fetal abnormality,
- If the woman is seeking an abortion due to trafficking, rape, coercion or harassment,
- Name and work address of the abortion doctor,
- Date the abortion was reported to DCS if the patient is a minor.
Voices for Life requests these reports each month from the IN Department of Health and reviews them for violations to Indiana code. Violations are then reported to the Attorney General’s office and the IN Department of Health. The AG’s office will pursue complaints relating to individual doctors, and the IDOH will pursue complaints relating to facilities. Examples of code violations that we find and report include:
- Performing a chemical abortion past 10 weeks post fertilization,
- Performing an abortion on a minor and not reporting it to DCS within 3 days,
- Failing to complete and submit a TPR within 30 days,
- Abortions performed at unlicensed facilities,
- Not providing the patient information and agreement paperwork.
We will also submit complaints about maternal deaths (as shown below), fetuses born alive, and abortion complications.
